Blepharoplasty, or eyelid surgery, is one of the most sought-after facial rejuvenation procedures owing to its remarkable visual impact and relatively swift recovery. It removes excess skin, fat and muscle from both the upper and lower eyelids, correcting the tired appearance, puffiness and deep under-eye shadows that many patients carry even after a full night’s sleep.

Beyond the aesthetic dimension, blepharoplasty also has a functional benefit: in cases with significant excess skin on the upper eyelid, this can obstruct the upper visual field, affecting driving or reading. Dr. Vricella assesses vision and tear production at every consultation to ensure a comprehensive, personalised approach.

The procedure is performed as a day case — the patient returns home a few hours later — with incisions carefully placed in the natural eyelid folds. Scars are virtually imperceptible after the first few months. It can be combined with a face lift, rhinoplasty or fat grafting for a more complete result if desired.

Procedure variants

The right technique
for you

Blepharoplasty can be performed on the upper eyelids only, the lower eyelids only, or on all four at once. The choice depends on the area of concern, the degree of ptosis and each patient’s aesthetic goals.

Upper Blepharoplasty

Correction of excess skin and fat on the upper eyelids. The incision is made in the natural eyelid fold so that the scar remains completely hidden. Indicated when the weight of excess skin creates a tired or heavy appearance, or reduces the visual field.

Upper eyelids

Lower Blepharoplasty

Removes fat bags and excess skin beneath the eyes. Can be performed via a subciliary incision (just below the lashes) or transconjunctivally — with no external scar — when there is no significant skin excess and only fat removal is required.

Lower eyelids

Full Blepharoplasty

Treatment of all four eyelids in a single surgical session. The most common option when excess tissue is present in both the upper and lower areas, delivering a balanced, harmonious rejuvenation across the entire periorbital region in one recovery.

Benefits

More than aesthetics:
quality of life

Blepharoplasty offers one of the highest benefit-to-invasiveness ratios of any facial procedure. A session of little more than an hour produces immediately visible changes that read as a natural rejuvenation: the person looks more rested, more alert and more expressive — without anyone suspecting surgery has taken place.

  • A fresher, more open and rejuvenated gaze from the very first weeks
  • Elimination of bags, under-eye shadows and excess skin that cause a tired look
  • Improvement of the visual field in cases with severe upper eyelid ptosis
  • Virtually invisible scar, concealed within the natural eyelid folds
  • Long-lasting results: the removed tissue does not return
  • Compatible with other facial procedures in the same session

The procedure

Surgical precision,
invisible scar

Blepharoplasty is performed in the operating room under local anaesthesia with sedation or general anaesthesia. Incisions are carefully planned along the natural expression lines: in the upper eyelid fold and just below the lashes on the lower eyelid. Through them, Dr. Vricella precisely removes excess skin, fat and muscle, then closes with very fine sutures that leave a minimal scar, virtually imperceptible over time.

  • Duration: 45 min (one pair of eyelids) to 1.5 h (all four)
  • Local anaesthesia with sedation or general anaesthesia as appropriate
  • Day case procedure: discharged the same day
  • Cold compresses and lubricating eye ointment post-op
  • Reading and TV possible within 2–3 days
  • Return to work in approximately 7–10 days

Your journey towards
the ideal result

01 — Consultation

Comprehensive assessment

Dr. Vricella examines your eyelid anatomy, assesses vision, tear production and skin quality. The most appropriate blepharoplasty variant is determined, along with whether combining it with another facial procedure would be beneficial.

02 — Planning

A personalised surgical design

Incisions are planned according to individual anatomy, the type of anaesthesia is selected and any procedure combinations are considered. You will receive full pre-operative information clearly and without pressure.

03 — Surgery

Day case procedure

Blepharoplasty is performed in the operating theatre at one of our clinics in Madrid, Marbella or Gibraltar. The procedure lasts between 45 minutes and 1.5 hours; patients are discharged the same day after a short observation period.

04 — Recovery

Swift return to daily life

Discomfort is minimal and easily managed. Most patients return to work within 7–10 days. Dr. Vricella’s team carries out scheduled follow-up appointments to ensure optimal healing and a natural, long-lasting result.

Frequently asked questions

Everything you need
to know

We answer the most common questions about blepharoplasty. If you have further queries, we are available to assist you during your consultation.

Blepharoplasty is performed in the operating room. Incisions are placed along the natural crease of the upper eyelid and just below the lashes on the lower eyelid, or via a transconjunctival approach in certain lower eyelid cases. Through these small incisions excess skin, fat and muscle are removed and the wounds are closed with very fine sutures. The procedure takes 45 minutes to 1.5 hours depending on how many eyelids are treated.
Blepharoplasty can be performed under local anaesthesia with sedation (the most common choice for patient comfort) or under general anaesthesia when combined with a more extensive facial procedure such as a face lift. The anaesthesiologist will recommend the safest and most appropriate option for your individual case.
Yes. Blepharoplasty is frequently combined with a face and neck lift, brow lift, rhinoplasty or periorbital fat grafting. Performing these together in a single session allows for a more comprehensive result with just one recovery period. Dr. Vricella will assess during the consultation whether a combined approach is appropriate for your case.
Most patients can read or watch television within 2–3 days. Return to office-based work is typically possible within 7–10 days. Moderate exercise can be resumed after two weeks, avoiding high-impact activities for the first month. Sunglasses are essential outdoors during the first few weeks to protect the healing tissue.
Discomfort is minimal. Patients typically describe a sensation of tightness around the treated area, tearing, mild light sensitivity and swelling — all well controlled with the prescribed medication and cold compresses during the first 24 hours. Strong analgesics are rarely required.
Contact lens wearers should wait at least two weeks before reintroducing them. Even then, some temporary discomfort may persist. Lubricating eye drops prescribed after surgery will be an important comfort aid during the initial recovery phase. Always consult Dr. Vricella before resuming lens use.
Scars are concealed within the natural eyelid folds and are virtually invisible. During the first months they may appear slightly pink before gradually fading to a fine, imperceptible line. From approximately two weeks post-op, make-up can be used to conceal any residual redness.
The tissue removed during blepharoplasty does not return. However, the natural ageing process continues: over time new bags or moderate skin excess may appear that could, in some cases, warrant a further procedure years later. For most patients the results are very long-lasting, typically 10 to 15 years, before any revision might be considered.
